Ultimate Nursing Services provides skilled care nursing as well as home health aide/homemaking services. We are Medicare and Medicaid certified and are capable of providing care under EPSDT, Adult Special Exception programs, private pay, and any of the existing waiver programs.
Experienced Case Managers have the ability to develop a comprehensive plan for each client to ensure proper access to services. Nurses are hired on a case by case basis, chosen from each region where the patients reside, to foster consistency, comfort, and a sense of community. Each nurse is trained individually to accommodate the acuity of the client.
Ultimate Nursing Services also provides an integration of services guaranteeing that proper communication with physicians is achieved and needed services are readily available. Prior to discharge from the hospital or facility, an effective plan of care is developed with the attending physician. Necessary equipment and supplies are established and a local network of professionals is comprised to ensure proper equipment maintenance and coordination of emergency services.
